Encrypted in transit and at rest, with clinical fields sealed under keys held in a managed KMS and rotated on schedule. A copy of the database on its own is worthless.

MFA on every staff account, roles scoped to the minimum needed, and emergency access that expires by itself, pages a human and leaves a record.

A provider reaches a record because the member granted it — for one purpose, and revocably. An employer never sees an individual's clinical record.

Reads and writes against a health record are written to tamper-evident logs, with alerting on unusual patterns and retention aligned to CERT-In directions.
